For mental health coverage, therapy must be considered medically necessary. For this qualification to be met, a diagnosis must be on file starting at the first session. You have a right to know and understand your diagnosis. Your diagnosis is part of your permanent medical file with the insurance company and cannot be removed once given.

Do I have a deductible for in-network behavioral health benefits? If so, how much?
What is my co-pay per session?
Is there a limit on sessions my plan will cover per year? If yes, how many?
Does my plan require pre-authorization for outpatient behavioral health benefits?
-
If I don’t take your insurance, I can provide you with a “Superbill”. Superbills are essentially receipts for services paid initially out of pocket, which you can submit to your insurance company as a claim.
You may be eligible for reimbursement if you have out of network benefits. Please contact your insurance by calling the number on the back of your card.
Here are some helpful questions to ask them on the phone call:
Do I have “out of network benefits”?
If yes: “What is my deductible?”
What part of that deductible have I met so far (if at all)?
What is my copay on out of network “behavioral health” visits?
How do I submit “Superbills” for reimbursement?
Do you reimburse for therapy with social workers?

You will be asked to place a debit or credit card on file for your appointments. You can pay for your therapy with Health Savings Accounts (HSA) or Flexible Spending Accounts (FSA).
You are not allowed to used these cards for missed appointments, though, so you will need to put a second card on file for these cases.
