A geriatric care consultant helps older adults and their families navigate the complexities of aging with clarity, confidence, and compassion. By providing expert assessment, personalized care planning, and trusted resource guidance, I support clients in making informed decisions about health, safety, living arrangements, and quality of life.

I’m a Geriatric Social Worker and a Therapist. Because of my diverse background & training, I bring a unique skillset as I strive to integrate holistic social supports through resource coordination & advocacy, identity the myriad of systematic barriers, while also understanding individual mental health needs.

Cultural Humility

My approach to consultation values the intersection of identity and culture. I practice cultural humility, the ongoing process of self-reflection and self-critique, recognizing and challenging power imbalances, and maintaining respectful and open attitudes toward individuals from diverse cultural backgrounds. Different dynamics of aging, including social expectations and norms are affected by one’s socio-cultural background and atmosphere. My work cannot possibly be one size fits all. Each case is conceptualized from a multicultural framework.

My Specializations:

Alzheimer’s & Dementia Support

With a deep & personal understanding of dementia & Alzheimer’s, their impact on cognition, behavior, and daily living, I provide compassionate guidance, tailored care planning, and practical strategies to enhance safety, communication, and quality of life. We can provide basic education that your doctor doesn’t have time to provide in a brief visit to help those new to their diagnosis, or suspecting it; the behaviors of decline and subsequent changes in personality, among other topics.

Aging in Place / Navigating Transitions

I assess home environments to support safe, independent living for as long as possible. I offer practical recommendations on in-home care, fall prevention, and how to balance autonomy with support. Then, I connect families with trustworthy, vetted resources — from home care agencies to benefits programs like Medicare, Medicaid, or VA services. I also help families understand what services are covered, how to apply, and what to expect. Whether you're trying to avoid unnecessary hospitalizations, prevent burnout, or find the right living option, I provide guidance that’s grounded, accessible, and responsive to your real-life needs.

Caregiver Emotional Support & Burnout Prevention

The caregiver experience is one of love and devotion, but also overwhelm, burden, stress, and often feelings of isolation. They experience burnout, health problems, time issues, financial struggles, especially if they’re the “sandwich generation”.

Helping caregivers learn and understand what they need, how to communicate with their family, learn boundaries, and how to prioritize themselves can help them and the family members they care for. I teach people to deal with the emotions– guilt, resentment, grief– whether they’re adult children or spouses. 

Trauma-informed Aging Support

Many older adults carry unresolved trauma or grief that resurfaces later in life. I bring a sensitive, trauma-informed lens to my assessments and care planning, especially for individuals with histories of abuse, loss, or medical trauma. I’m attuned to the unique ways depression, anxiety, and isolation can show up in older adults — and I provide compassionate support, referrals, and education to help families address these challenges effectively.

Holistic Assessments & Personalized Care Planning

II start with a thorough assessment to understand how much support an older adult needs at home, identify safety concerns, and evaluate overall well-being. I also take time to assess the needs, stress levels, and capacity of caregivers — recognizing that their well-being is essential to sustainable care. Whether it’s help with daily tasks, memory-related changes, or navigating a new diagnosis like Alzheimer’s, I provide clear, personalized recommendations. From in-home care and safety planning to dementia-specific resources, I guide families with practical solutions and compassionate support every step of the way.

Consultations can be “one off”
1-hr sessions

Get some answers, direction, or support & advice for how to move forward. Afterward, I can offer to make recommendations about what I feel is prudent & most beneficial in your situation. I can make these reports in writing for future use if you’re interested.

I don’t offer Case Management at this time

Case Management or Care Coordination can include things like communicating with doctors or other providers, coordinating in-home care or services like physical therapy, hospice, or home health, briding gaps between hospital discharges, rehab stays and home care, or acting as a point person and making follow up calls.

Unlike clinical consultation, which focuses on providing expert advice and guidance on specific clinical issues, case management involves ongoing service coordination and support across multiple areas, which I don’t have time to do, unfortunately.

I may offer case management services in the future.
